I would say that he has a rather out there way of thinking, or simply he is crazy, though perhaps he is sociopathic. Moving from personality the most obvious thing he is is a murderer, and a guilty one at that. Though he does not seem to understand he is feeling guilty, that is what causes him to hear the beating of the heart, the loud noise in his ears. In the end it's hard to tell whether he really felt the guilt or simply thought that the police had found him out.
Though if he had had the sense to remain calm as they spoke, he would have never been found. The irony really is how he argues in being crazy, yet in the end, this simply makes him sound even more insane.
The narration is in the first-person. The story is being related from the point of view of the murderer in the tale, who becomes increasingly overcome by subconscious guilt over his actions.

Other than the title, The Telltale Head being a play on The Telltale Heart the similarities lie in The Telltale Heart having the narrator, who is presumably the murderer, being haunted by the sound of the victim's beating heart. Bart, who is also the narrator of The Telltale Head briefly, is haunted by the voice of Jebidiah Springfield.
