The only way I know to crochet letters onto an already completed afghan would be to use surface stitching.
Another method, which is not crochet but which would give a very nice look, would be to cross-stitch them on. This can be done on single crochet or tunisian crochet (afghan stitch).
Another way to get letters on an afghan would be to crochet strands of chains or single crochet strips and shape them into the letters needed, then attach them to the ghan. That would give a 3-D effect.

There are several different ways to crochet names into or onto a crocheted piece. One way would be to finish the piece and cross-stitch the name on it afterwards. This would work best IMO in afghan stitch, but works okay in single crochet as well. Another way would be to use a different color to make the name stand out. Depending on what you want the back of your work to look like, you can do this in different ways. You would have to either carry your yarn along the back if your color changes are not too far apart, you could work over the second color, always letting the color you are not using sit atop the stitches, or you could cut the yarn at each color change. Those tails would need to either be crocheted over or woven in later. Yet another method of getting a name in a piece is to work it in filet crochet, using either solid blocks or open blocks for the letters depending on the finished look you desire. You could also embroider the name - for example if you wanted to "sign" your piece, you could embroider your initials in a corner. To learn how to work these methods you might consider typing "crochet pictures" into a search engine. There are some who have mastered the techniques and have created great videos to show you how.
To create your own DIY ergonomic crochet hooks, you can use materials like polymer clay or foam to mold a comfortable grip onto a standard crochet hook. Simply shape the material around the handle of the hook to fit your hand comfortably, then let it dry or set before using it for crocheting.

A whip stitch in crochet means to thread a yarn needle with yarn and "sew" essentially. For instance, if you make an applique such as a truck, and wish to put it onto a blanket, you would thread your yarn needle with matching yarn, begin underneath, and stitch it to the blanket as you would sew a patch onto a pair of jeans. It also means to sew two peices together-as in if you have two straight edges like two granny squares to make an afghan, to stitch the two together.

To make lettered shirts, you can use iron-on letters, fabric paint, or embroidery to add letters to a plain shirt. Iron-on letters are easy to use and can be found at craft stores. Fabric paint allows you to hand-paint letters onto the shirt. Embroidery involves stitching letters onto the fabric. Choose the method that best suits your skills and preferences.
