It's a relatively simple concept. One sticks a needle into the skin deep enough to get the ink under but not too deep that you reach the muscle. I just got one done this summer, and what my "artist" did was tie a needle to the end of a pencil as his tool. Then he poured some ink into a bottle cap and had some paper towel ready. Just dip the needle (it should be clean and sanitized!) into the ink and start poking away! They take a longer time, obviously because the pokes are one by one, as opposed to a machine that does a lot of pokes super fast. Irecommend having someone who has a) gotten a tattoo before so they'll know the depth of the poke and b) confidence. Mine was done by a dude who has loads of tatts and had done some touch up work on some of his own. If you want an elaborate tattoo with colors and that's really big, stick and poke is definitely not the way to go. Small, simple and little to no fill-ins are really what work.
