How do you import custom items into hammer editor?

Well you could mean several things in my opinion:

Props / Models, Textures / materials and then entities.

Firstly If you Map for a particular game, then you should select the correct configurations in Source SDK, most games should already be set up, so browse through the engine version list at the bottom and find your game, I've gone and set up a garrysmod configuration as that is what I map for.

To set one up: Click edit game configurations in utilities and click add, then name it and select the game directory, this should be in:

<Your Steam Directory>\steamapps\ Steam Username>\

The game directory you select should have folders called: maps, materials and models, and also have a text file called GameInfo.txt.

To import custom props, put the models and materials used for the prop in the correct folders inside the directory of the game you selected. E.g. For Counter Strike: Source the directory would be:

<Your Steam Directory>\steamapps\ Steam Username>\counter-strike source\cstrike\

Then in the materials folder for the materials and same for models.

*Note: You can also put a new folder inside the materials, or models folder, as long as the model points to the material, if you don't understand just stick it in materials directly*

Importing materials is exactly the same except without the models files.

And entities you have to script them and I'm sorry I don't know how!
