Typically, patient stats and history are at the front, along with legal documents -- POA's, living wills, etc. followed by a record of each visit or procedure and the doctor's notes. If you're really dilligent, you'll even include phone calls, again in chronological order. If this applies to you, it's probably wise to keep psychotherapy notes separate from the clinical notes. Then store them in a filing cabinet which is secured from moist as medical records do need to live a long time. If you also can, you can start transferring your folders to a computer and keep all records in it.

It is possible for the medical records to stay in the room with the patients. They used to take the patients clipboards and medical records on the outside of the door, but now they do not do so. It is has actually been discontinued a long time ago due to security reasons. People would take medical records of the patients in a room and dress up as a doctor and create fake badges and be impersonated as a doctor. So, no, they cannot keep the medical records in a room with a patient.
