The pair of button hole scissors I own has a measuring scale on the lower blade so it is easy to adjust the screw to set your cut. On a shirt you want the button holes to go in a top to bottom cut line. You mark the midpoint of all your holes and make a fold at a right angle to the line of your cut on that midpoint. Then you cut on the fold so that you are cutting through 2 layers but when you are done and unfold you have 1 straight cut. Since mine are marked with a scale, if I set it at 1/4 inch then when I unfold the fabric I have a 1/2 inch cut.
