The third brake light is accessed from the top. Push the third brake light housing about 1/2" to the left, toward the passenger-side, and then lift up on it to release it from the rear package shelf. The wiring has enough slack that you can pull the housing out enough to work with it. Then give the bulb housing a quarter-turn counter-clockwise and pull it out of the housing. The bulb pulls straight out of its housing. The correct replacement bulb type is a 7443 which is the same bulb used for the stop, turn, and tail lights.
