There are several things that can cause your transmission to slip, while first pulling out. The most common cause is a worn drive gear.

You could be low on transmission fluid. The same thing happened to me.
Transmission slips in and out of gear on and off acceleration, the trans. fluid is burnt, the fluid level is low and the unit has tranny fluid leaks.The vacuumn modulator valve needs replacement if its pulling trans fluid through it and into the intake/exhaust manifold and causing slippage in the trans.
You might need to get your clutch replaced. Or your transmission is about to fail. Go to an auto mechanic for diagnosis and repairs.

You probably have a slipping clutch band in your transmission. Take it to a transmission shop.
Check the color of the transmission fluid. If it smells burned and looks dark that is a bad indication. If the trans hesitsates between shifts or slips when you are driving those are very bad indications. Taking a while to go into gear is indicative of worn seals.
When the cold transmission slips when shifting in the second gear, it means that either the fluid level is not correct or the filters are faulty. When this happens, your gearbox may need extensive repair or may need replacement.
