It would depend on what you mean by 'jerks.' It could be a fuel issue, tire alignment, ect. If it's really bad, it's probably best to take it in.

i have a 95 es300 but it doesnt jerk when i change gears when i ride it jerks but i found out it was a motor mount front motor mount which will cost bout $112 to replace but if when you change gears it could be your transmission slipping that cant be goo but check your motor mounts first....

When the mount is broke off the engine you will hear a loud clicking sound from then engine being unstable and shifting around. Listen close enough and you can hear it shifting when the vehicle jerks.
