No you don;t need to pull off the door trim to access the stock speaker. You just need to remove the door handle and the grill section. Installing a larger speaker is possible but requires more work as you need to remove the door card and make a custom mounting plate for the speaker itself Try to use Toyota speaker adapter plugs so you don't need to cut wires and mess the electric system up. You can get them from Ebay or Amazon.
mine has 6 1/2 but they need adapters for new speakers....

it takes a 61/2 in the rear and a 51/4 in the front door panels I have a 1995 Toyota Corolla and the front door speakers are 4" across the cone, hole to hole is 5.25 measured diagonally.
When i remove mine, i typically stick a screwdriver at the very bottom of the door, up into a small opening below the speaker section. Simply push it in there and slightly inwards and itll pop it right out!
