Start with a tune up.Suggestions are spark plugs,spark plug wire set,distributor cap,rotor button,fuel filter,pcv valve,air filter.I would recommend cleaning the carburetor if equipped with that(should be on year model) if not do a fuel injector cleaning.Also removing the egr valve and cleaning with carburetor cleaner or brake cleaner and sand paper or wire brush and make sure you reinstall the metal gasket exactly as it was on this.If the oxygen sensor is OLD over 60,000 miles replace it and if the catalytic converter is old it may be clogged up due to carbon.I would replace it if it's got at least 60,000 miles or more due to age and wear this prevent the truck from breathing.Not to get to indepth but these trucks have a exhaust collector which is connected to the exhaust manifold and has screens inside it which over time get blocked up with carbon which will choke the engine.To fix this requires removing the exhaust manifold and charge air pipe assembly and connections below the engine.To clean this collector use brake cleaner or soak the part in muractic acid(be very carefull and use protective gloves) to remove and then clean the residue with baking soda let air dry and or blow out with air tool and reinstall.Start simple and then if no change pursue the more timely things and costly from that point.
