they are 2 completely different substances usually- If they combine they make a compound or a molecule. this is 2 substances together.

Example-

H2O=Oxygen+2 Hydrogin atoms(water)

No. Elements are not created or destroyed in any chemical reaction. Combining elements chemically can only ever form a chemical compound. The nuclei of the elements are never affected.
