Challenger had a damaged O-ring on one of the SRBs (solid rocket booster) which led to the explosion of the liquid fuel tank and the destruction of the shuttle just over a minute after launch. Columbia broke apart on re-entry due to a damaged heat shield tile which was struck by a piece of insulation on take-off.
