I'm pretty sure it's four Daleks in the Cult of Skaro.
Fourteen times in the original series (the first eight Doctors) - twice with the 9th Doctor, three times with the 10th Doctor. Also, at least three times with the 11th Doctor (counting seasons 5, 6 and the first episode of 7). That makes a total of 22 times from the beginning of Doctor Who to the beginning of the seventh season (of the modern one). And when I say 22, I mean 22 story-lines that feature the Doctor and Daleks. Not 22 episodes.
