Yes the only one to my knowledge.
Each Apollo mission carried 3 crew members.
1969 (november 14 was the exact launch day)
Three crews went to the moon in 1969, Apollos 10, 11, and 12. Apollo 10 was a "test run" for the moon landing and took place in May, 1969, but did not actually land. Apollo 11 was the first lunar landing (July, 1969), followed by Apollo 12 (November, 1969). Apollo 10 crew: Tom Stafford, John Young, Gene Cernan Apollo 11 crew: Neil Armstrong, Buzz Aldrin, Michael Collins Apollo 12 crew: Pete Conrad, Alan Bean, Dick Gordon
The crew of Apollo 12 consisted of Charles "Pete" Conrad, Richard F. Gordon, Jr., and Alan L. Bean. Conrad served as the commander, Gordon as the command module pilot, and Bean as the lunar module pilot.
The reason was because Apollo 13 followed Apollo 12.
The crew of Apollo 12 which visited the Moon in November 1969 were are US Navy personnel.

Apollo 12 was launched like all spacecrafts , from Cape Kennedy Florida.

Charles "Pete" Conrad (Commander), Richard Gordon (Pilot of the Yankee Clipper command module) and Alan Bean (pilot of the Intrepid lunar module).
No, they were not - the first moon landing left Cape Canaveral in Florida on 16th July, 1969 with the crew of Apollo 11 landing on the moon four days later. Between July 1969 and December 1972 there were (in total) six manned moon landings made by Apollo 11, Apollo 12, Apollo 14, Apollo 15, Apollo 16 and finally Apollo 17. Apollo 13 was forced to return to Earth when an explosion in the rocket occurred when the crew were halfway to the moon but fortunately the crew survived and returned to Earth safely. There is even a movie about the Apollo 13 moon mission so that's evidence the moon landings were not faked.
Yes. Alan Bean and Charles Conrad walked on the moon as part of the Apollo 12 mission.
The Apollo 12 landing crew consisted of Charles "Pete" Conrad and Alan Bean.
