To avoid getting lost ion space an astronaut should always hold their mommy's hand. If they do get lost they should retrace their steps carefully to where they were when they last saw their spaceship.

Astronauts, when they go out into space, tether themselves to their ship and carry small, hand-held devices that can shoot out concentrated streams of air. That way, even if the tither does break, they can propel themselves to the ship. As to what happens if they get lost, if they cannot communicate with their ship, nor see it, they're generally dead unless they regain communications. (Oh, and personally, if I was lost in space, I would scream. But that's just me. Or propel myself into Earth's atmosphere.)
