The only reason a washroom would be blocked is if there has to be maintenance done on the toilet or another portion of the washroom (generally not the case) and more likely it is a group of people at the person's home and more than one person needs to use the washroom. In any case if you can't hold it don't be shy and ask the person in front of you if you could please get into the washroom as you don't think you can hold it and then make a small joke about it. Humans are just that and can understand that concept. If you are a young person and there is no way to get into the washroom you may have to leave and find a bush or shrub outside, but be wary .. if caught by a neighbor you could be reported for indecent exposure to possibly lurking around in hopes to break and enter.
