The moon is big enough to have its own gravitational field, so it really can't fall apart unless something really spectacular happens. It'd require a HUGE impact/explosion to overcome that gravitational effect to the point where the moon would actually split into separate pieces. Bad, bad news for us, the next door neighbours.
If the moon fell out of orbit, it would either crash into Earth or be slingshotted away into space, depending on the angle and speed of its descent. Either scenario would have catastrophic consequences for both the Earth's environment and life forms.
If you were on the moon your weight would lower, as there is no gravity.
nothing would happen
the grass will die
If the moon was on the left and the sun was on the right, it would suggest that the moon is in its waxing phase moving towards a full moon. This positioning is a natural occurrence based on the moon's orbit around the Earth and the Earth's orbit around the sun.

then there would be no moon

You would see the moon.
you would jet a bit burnt if you where on the moon. I'll find out about the next eclipse
If the Earth magically became a black hole, the moon and other satellites in orbit around Earth, would be literally ripped apart into tiny bits and swallowed by the black hole.
they would stay on the moon and they may not decay.
A full moon
The moon phases would change.
It wouldn't fall, per say. The Moon would crash into the planet Earth, ending all life.
Earth would blow up along with the moon.
There would never be a full moon.
