Ken Mattingly was taken off the Apollo 13 mission due to concerns about potential exposure to German measles from a fellow astronaut. NASA decided to replace him with Jack Swigert to prevent any health risks during the mission.
The flight was commanded by James A. Lovell with John L. "Jack" Swigert as Command Module pilot and Fred W. Haise as Lunar Module pilot. Swigert was a late replacement for the original CM pilot Ken Mattingly, who was grounded by the flight surgeon after exposure to German measles.

The three members of the Apollo 13 were: (Commander) James A Lovell, (Lunar module pilot) Fred Haise, and (Command module pilot) John L 'Jack' Swigert. The command module pilot was originally Thomas 'Ken' Mattingly. However he was exposed to German Measles and so was unable to go on the mission. He was replaced by Jack Swigert. Interestingly, Mattingly never did get German measles, but did play a very important role, along with many others, by working with ground crew to help them devise a plan to bring the crew back safely.
Retired Astronaut Rear Admiral (UH) Thomas Kenneth "Ken" Mattingly II, scheduled to be the Command Module pilot for Apollo 13, was exposed to Rubella (German Measles) just 3 days prior to the mission launch. He was removed from flight status by the mission Flight Surgeon as part of normal safety procedures and replaced by Astronaut John L. "Jack" Swigert. Though he never contracted Rubella, his contributions in helping the Apollo 13 crew conserve enough battery power to restart the Command Module systems prior to re-entry, along with the teamwork of the entire Mission Control team, was key to their successful return to Earth after the explosion that crippled the Apollo 13 Service Module.Mattingly eventually did make it to the Moon - he flew as Command Module pilot aboard Apollo 16 just 2 years later, and also flew the Shuttle aboard flights STS-4 and STS-51C.
