It is determined by how many balls are pitched to you. It has nothing to do with the score. Step 1 is the first four balls. Step 2 is 6 balls, and so on. I just counted step 7 and it is 9 balls.

You dont. The screen seems to have an implication that you do since it shows varying levels with varying numbers of moons for the game. 12 is what it is currently playing on but it seems to imply that higher levels will go to 16 moons then 20 etc.
