Every author has a different way of working. Many authors have several ways of working, depending upon the reason, the inspiration, the assignment, the style of his/her writing, the subject matter, the goal, the journey. The variations in how one organizes a "passage" (and I'm not quite sure what you mean by, "a passage,") are so numerous that it's really impossible to answer. You could ask an individual author that question and perhaps get a more definitive, however personal, answer. How process is taught in school, writers' workshops, genres may have little to do with how each author finally develops and arrives upon his/her individual method or methods of working. And that, too, continues to evolve with time and experience. Maybe this isn't too helpful. But it's the best answer I can come up with. Go and ask around; approach the writers you know or find and you may learn at least a sampling of what is entailed in an author's creative process. I could describe my own methodology, but it would take at least double the length of this post.
