1. block rear wheel so it won't roll; 2) loosen lug nuts; 3) jack up wheel with jack under cross frame; 4) place jack stand under spring support & lower jack to rest PU on stand but keep jack in place holding some weight; 5) remove lug nuts and wheel; 6) remove the 2 bolts that hold the brakes to the brake mount, with wire hook brake onto spring; 7) remove screw(s) if so equipped from front center of rotor then pull rotor off. That's it if I recall correctly.

It's called "brake chatter", and it's caused by warped brake rotors. Replace the rotors and the problem should go away.

remove your tire,take off your brake assembly,two hex head bolts,remove old rotor,and put new one on.
You have not set and adjusted them correctly.
Front or rear rotors and 2 wheel or 4 wheel drive, it makes a difference.
