When you go to the DMV to get your CDL, you'll be expected to show either your DOT medical card, or the DOT physical long form. Additionally, any employer hiring you as a CMV operator is going to require you to present your DOT physical card, and possibly the long form physical, as well.

The expiration date should be both on the long form and your DOT medical card. It can be valid for up to 24 months, but can also be for less in cases where age, medical condition, etc. becomes a factor. The expiration date is the day it expires - there is no grace period.

Find a medical clinic in your area which does DOT physicals, get a DOT physical done, show your new DOT medical card to the DMV. It's that simple.
The doctor should have them on hand (both the long and short forms) when you get your DOT physical.

If you are able to locate the medical provider who issued you your DOT physical card or his office, he or she can reissue you another card providing it is not expired. If you are unable to locate that provider then you will have to undergo another physical.
Your license won't be suspended, but you'll be forced to downgrade. You can't hold a CDL if you can't pass a DOT physical. And if you can't pass a DOT physical, you have much bigger problems than your license to worry about.
