Remove wheel and disconnect outer tie rod from lower control arm. Clearly mark original position of outer tie rod end on shaft and remove outer tie rod end. Remove rack and pinion boot revealing. Unscrew inner tie rod from rack and pinion assembly.

A rack end is the part of a steering system (rack-and-pinion-steering) that connects a steering rack to the tie rod (track rod/connecting rod) which connects to the wheel. In a simple steering system, there are four parts: steering rod - rack (this is the one that works with the pinion gear) - rack end - tie rod
A socket set and a set of end wrenches is all you need.
Once a Rack starts leaking it's time to replace it. A leaking rack & pinion system is usually caused by worn bushings that beat on the seals. If your Taurus Rack system lasted this long, consider yourself fortunate. Many don't last this long.
The ball joint should be permanently in the tie rod end. If it is bad, you have to replace the entire tie rod end.
Replace either the inner tie rod end or the outer. Which ever is worn out.
Yes, you can replace just the "boots" on the end of a rack-and-pinion steering provided that the mechanism is not damaged. Unfortunately, the boot is just a dust cover and if the inner mechanism has started leaking there really isn't much that can be done without completely disassembling the mechanism. The fluid in a rack assembly is under pressure from the Power Steering pump and once it starts leaking the rack is pretty much "done".
