First off can you feel it shift through all the gears while off (not running). Second off while running can you shift through the gears or does it grind while trying to shift through them. If it grinds when you try to shift then your clutch is shot. Good luck
if you are missing any gears in your manual transmission it is shot. You might as well replace the clutch while you have the tranny pulled so you dont have to do it later.
About $2000 if you have a rebuilt put in. More if you have a new one put in. You can get a used one from the boneyard (salvage) for about $550 and it is about $500 labor to get it put in. If you are having problems with the manual transmission, take it to a shop to make sure it's not the clutch that has gone out. It won't be any more or less labor, though, because the transmission has to come out to replace the clutch.
Whenever you are having a problem with the clutch disengaging the engine from the transmission it is advisable to bleed the clutch slave cylinder. If the clutch can't be operated by bleeding the slave cylinder then the clutch master cylinder is gone and should be replaced.

It means the reverse clutch pack has gone out in your transmission and needs to be rebuilt or replaced.
first check your clutch slave cylinder for leaks, if its leaking ,replace and bleed system,if its not leaking,bleed system ,dont forget to bleed clutch damperand if its still not right, then replace clutch master cylinder.
well could be throw out bearing gone,cable broke,probably worn out clutch.Check to see if clutch has oil resv. for manual clutch,slight possibility it is out of fluid if never been topped up.
Well you have a strange issue. Being the trans will not shift into any but reverse would point to the clutch being toast. Normally if the truck will not shift is the slave clyinder or even the clutch master or throw out bearing gone. To rule out the the throw out did it noise while the truck was in neutral and even while in gear.If either apply may be the throw out is gone, but sounds 100% clutch is gone. If the truck will shift into soley reverse then this would rule out the slave, and clutch master cyldr and even the throw out in most cases due to the it is moving the fork to engage the clutch and the surface of the clutch is worn to it's life except reverse due to it is not used as much. I recommend a new clutch and replace the throw out bearing and would be a good idea to replace the pilot bearing and go ahead and do the rear main seal even if its not leaking if its got lots of miles, plus the age would make me do it being you have the trans pulled out.

Depends on the year of the truck It could be a computer control problem. This computer could be in "Limp mode" because a sensor is bad or a control inside the transmission is bad. It will need to be checked by a knowledgeable and trust worthy person.
