On cold days, you should let the engine warm for a minute or two. Then, if you press the accelerator to the floor for just a second and then let it up. This is done really fast. Floor it and let it right up. The fast idle should release, and hopefully you won't get that kick any more. You may have to press the accelerator down more than once to get the idle to release. Duramax elevated idle programming: 1) Turn the key to ON. 2) Push accelerator pedal to the floor. 3) Push the brake pedal 3 times. 4) Release accelerator pedal. 5) Start engine. If it is below 32*F/0*C (and some other conditions), it will display Elevated Idle and will do it until engine warms up, you press a pedal, or put it in gear. (basically) It will not do it above 32*F. It will stay programmed for the future. To turn it off, do the same process.

Not really an answer, more to the question. You can hear the knock at idle. However, if the engine has time to cool, it's not so audible. Also, if you hit the accelerator, the sound seems to die away when the engine returns to idle. The sound is coming from both the top and bottom of the engine.
