Means you have water in system. There is a pull type drain cable left of center under the hood that looks like a transmission dip stick that should save fuel water drain right on it. If you pull it fuel/water will drain out the bottom end. Make sure you push back in because if has'nt been used chance ar it will stick. probably time to chance fuel filter located in valley between heads and behind fuel distributor.

On a 2001 Ford E350 with a 7.3L diesel engine, the fuel filter is located on the driver's side frame rail, just before the fuel tank. It's typically mounted in a vertical position and can be accessed from under the vehicle. Make sure to relieve the fuel system pressure before attempting to change the filter to avoid spills.

IDI diesel fuel filters are (looking from the front of the vehicle) on the right side of the engine. Looks like an oil filter hanging upside down. On powerstroke diesels, they are on the top of the engine, mid-way back. They have a black, "screw-on" cap about 4" in diameter. Recommend using a WIX filter... STAY AWAY FROM THE FRAM FILTER!!!

Depends on which engine it had... gas and diesel engines were available for that vehicle.
Im not sure about 2006...but 2005's have two...one under drivers side by frame and one on top of engine in the fuel filter housing assembly. You will have to remove a few things on top to access filter, but the one underneath is simple.
The diesel engine that would be in a 2008 Ford E-350 van would be the 6.4L Powerstroke Turbo Diesel V8. This Powerstroke engine got poor fuel economy with about 16mpg highway and 12mpg city.
