This depends on what type of vehicle you own. Generally speaking you going to have to jack up the vehicle and pop the brake fluid reservoir lid. Next remove the wheel, then you will loosen the caliper bolts and remove the caliper. DO NOT LET IT DANGLE BY THE BRAKE HOSE. The pads will be visible for removal and replacement. The break rotor replacement depends on if your reusing the old machined rotors or new ones. Add anti-squeal to back of new pads and let dry. The reassembly as you disassembled. Then start vehicle and repeatedly pump brakes until normal break feel returns. Then wear in the new pads according to manufacturer's instruction.
