Depending on your vehicle it could be a bad temp sensor, a bad relay(s), a bad switch, faulty wiring, a bad fan motor, a faulty ECM,
The cooling fans on a 2002 Grand Am GT might not be working because of a problem with a fuse. There is also a temperature sensor in the engine that tells the fans when to come on. This sensor may be bad.

The engine temperature must reach 232-235 degrees in order for the cooling fans to come on. They will shut off at about 195-200 degrees. Also when the air conditioner is on the cooling fans automatically come on regardless of temperature.
