Helena does indeed want Demetrius to love her, but at the same time she doesn't believe that he will. Remember how she called herself "as ugly as a bear"? So when, thanks to the love potion, Demetrius starts to say he loves her, she thinks he cannot be serious, and that Demetrius is just doing it to make fun of her.
Helena is Miss Low-Self-Esteem. According to her, she is as ugly as a bear and deserves to be treated by Demetrius in the same way he treats his spaniel. So when Lysander and Demetrius come along professing their love for her, she thinks they must be making fun of her because she is so much less attractive than Hermia. Then along comes Hermia, who is bewildered by Lysander's change of behaviour. Helena does not see this for what it is. She assumes that Hermia must immediately see that Helena is being made fun of, and if she is a good friend she would take Helena's side. Since she doesn't take Helena's side, Helena immediately comes to the conclusion that Hermia is part of this imaginary plot against her, and she gives Hermia a long lecture about being a better friend. Hermia expresses her incredulity: "I am amazed at your passionate words. I scorn you not; it seems that you scorn me." As crazy as Helena's logic seems to be, it is just the kind of reasoning you find in people who are convinced of their own worthlessness.
This is indeed a scene which is great fun and is difficult to do badly. It is extremely entertaining in any movie version you should care to watch. The reason for this is conflict. After Puck explains to Oberon how he has used the love-flower on Titania and "the Athenian", we see Demetrius and Hermia arguing. He still is trying to get her love, and she is trying to find out what has happened to Lysander. Apart from their conflict, there is conflict between Oberon and Puck over Puck's error in love-juicing Lysander instead of Demetrius. Oberon juices Demetrius and he falls for Helena, Hermia arrives and then the fun starts. The following conflicts are all going on at the same time: Demetrius protests to Helena that he loves her but she thinks he's making fun of her. Lysander also protests to Helena that he loves her and she thinks he is also making fun of her. Hermia protests to Lysander that she loves him, but he says he hates her. Hermia gets mad at Helena for stealing Lysander's love and Helena thinks Hermia is also making fun of her. Finally Lysander and Demetrius fight over who should get Helena. In the course of the scene every one of the four lovers finds him or herself in conflict with each of the others. This crazy madness only ends when Puck comes along and puts them all to sleep.
Many people think they are so alike you cannot tell them apart, but there is a difference. Without the assistance of the love flower, Lysander is a true lover, and a very considerate one. He does not take advantage of Hermia when they are alone in the woods. He is prepared to sacrifice his life in Athens to exile himself with Hermia. Demetrius is more changeable. He has dumped Helena so as to have a chance with Hermia, even though he knows Hermia does not love him. He is quite happy to go along with Egeus's plan to force Hermia into marrying him. He threatens to rape Helena when they are alone in the woods, and only desists when she says that it won't be rape because she will be consenting. During the Pyramus and Thisbe play, poor Starveling gets heckled right out of his text by guess who? Demetrius, aided and abetted by Theseus. All Starveling had to say was "horned moon" and away Demetrius went with the dirty joke. Lysander does not take part in the heckling and encourages Starveling, saying "Proceed, moon". Lysander is much kinder; Demetrius is rather mean. Indeed it appears that Shakespeare, in drawing the characters of Demetrius and Helena, was portraying an abusive relationship. Helena is a person of low self-esteem ("I am as ugly as a bear") who feels that she deserves, unworthy as she is, to be mistreated by her lover ("I am your spaniel.") Demetrius is unfortunately one of those weak and cruel men who is happy to oblige.
