Really means "an actual truth or fact".

Very means "in a high degree".

For Example:

"Now it's raining here, really." --- Here we are informing the actual fact or the condition of raining in this place with more importance.

"I like Ice-Cream very much." -- Here we are informing the liking of Ice-Cream more than to the usual snacks with a high degree.
