Almost any tattoo shop will draw up a design for you and make a stencil. Some artists will draw the tattoo design on with a pen instead. You as a client don't need to provide a stencil.

Bloodlining is used when people dont want an outline for their tattoo (like a smoky effect). It also comes in use during white ink tattoos, because a stencil ink would mix with the white ink and corrupt the tattoo.
Yes lip tattoos are permanent. They still use ink and a tattoo gun for them. Although your tattoo wont be seen if dont on the inside of the lip its still a very painful process

Well generally we would use a drawing on a light box to take an outline from a design that you would bring in, assuming it would be a non custom piece. besides that we would use a thermo fax or hand trace the design on carbon paper to get our stencil
There are tattoo removals but even after that surgery I believe there is still a scar that remains..Otherwise naturally no tattoos dont fade away completely. They do start to lose color though.
