Tongue barbells need to be removed and cleaned, even more so with internally threaded barbells** (**these are barbells where the ball has a threaded post in it and it threads into the shaft ) these barbells have a hollow void in the center and can draw in body fluid and bacteria and allow it to grow inside the barbell shaft. This bacterial growth will cause repeated infections until the jewellery is removed and cleaned or removed and replaced with new jewellery. Please keep this in mind, all jewellery needs to be removed from piercings and given a really good cleaning every couple of months over an above the normal cleaning you give it when bathing.
