At a professional piercing parlor, they usually don't. You can actually easily numb the area yourself by holding ice to it for 10-20 minutes before (maybe even less). There are creams you can get from your doctor, but they might not be good for the lip.

Yes, you can get freezing for your lip. but then again it all depends on the piercing artist and location i suppose. I have my lip done and I did get freezing without even asking for it.
It depends on the shop. Some shops do it, some don't. It's just a lip piercing, only takes a second, you can do it :]
