Well you need a business license and a health inspection but before you go running off to get those you must have documented proof of training as a body piercer, CPR and first aid certification as well as blood borne pathogens training. This isn't something you can just fly into you need to pass the requirements the city and local health protection branch have in place for the protection of the general public.
Be ready to spend a lot of money before you get close to getting a license and health permit. Proper training can run you up to $5000.00 just to get the certification and training to do piercing, then you have facility costs that can run you another $5,000.00 for gear and then the site preparation costs can be another $10,000.00 +. Most city licensing branches will not allow you to operate from your home.
Professional body piercers who own there own studios pay a lot of money out to get where they are, there is no short cut, you wanna run with the big dogs you better be ready to shell out. It's all about providing our clients with only the best materials and services in the safest manner possible. My recent studio has cost me $20,000.00 just in renovations and construction costs, mind you I had all the equipment already in place.

Yes you would need a valid drivers license to register a vehicle in mass.
it depends on what kind of ear piercings you'll be doing. if you'll be working at like...a wal-mart, claire's or something like that, they'll ususally give you some sort of training in-store with the piercing gun (which is really not good for piercing AT ALL). but if you'll be using hollow needles to do more unusual ear piercings (including regular lobes) you're gonna need a piercing license.
