By the 'sand of time' I am going to assume that you mean the hourglass...This is common amongst reaper tats and many others...The meaning is obvious....Time is running out for all of us, and the hour glass is a symbol of that....Also it can mean to make the most of the time we have left...
Strangely enough a clock may not mean the same thing tho, it may mean that time is stopped, or delayed by a broken heart or prison...
