When you can no longer feel the disc portion on the inside of your lip, in most cases if the disc is descending into your lip you should be able to see your piercer and he/she can usually get it out with a little effort. Then placing a larger disc labret stud in the piercing will resolve the creep over effect that sometimes happens. If the disc is fully covered and there is no sign of an entry point inside the mouth then surgical intervention is needed.
