Why is MacConkey agar inoculated with Urine specimen?

MacConkey agar medium differentiates between the lactose fermenting and nonlactose fermenting bacteria. Most of the bacterial commensals from our large intestine are lactose fermenting, Escherichia coli being one of them. When bacterial growths and colonies on MacConkey agar is pink coloured, then the organism is fermenting lactose from the medium, on the other hand, if transparent/transluscent, colourless growths and colonies appear on the medium, the bacterium does not ferment lactose. The flushing action of urine, make it sterile in an uninfected healthy person. In an event where the patient is suspected to have urinary tract infection (UTI), urine of patient is inoculated on MacConkey agar medium and incubated at 37 degree centigrade overnight. In most of the cases, UTI is caused by Escherichia coli that is lactose fermenting and produces pink coloured colonies on MacConkey agar medium. Although the urine is sterile, it may get contaminated by the organisms of skin microflora or by surrounding microbes due to improper collection of urine. In such cases, UTI is diagnosed by counting bacteria in urine sample. For significant bacteriuria (bacteria in urine), the count must be equal to or more than 1 lack bacteria per mL of urine. What are the abnormal constituents of urine?

Abnormal urine can have:CarbohydratesProteinsketone bodiesBloodBile saltsBile pigmentsFatsCARBOHYDRATESCarbohydrates present in it such as lactose and fructose.Physiologically in pregnany and lactation, lactose is excreted in pregnant women.Pathologically: if lactose is excreted in the urine, then this is due to lactose intolerance.Glucose could be excreted in the urine in Diabetes mellitus, glucosuria, renal glycosuria etcFructose in essential fructosuriaGalactose in galactosemiaPROTEINS:Ecretion of proteins in urine is due to:1- Increase in the permeability of the glomerular capillaries as in nephrotic syndrome etc2- Damage to the capillaries membranes as in glomerulonephritisFollowing proteins can be excreted in abnormal urine:AlbuminGlobulinHemoglobin (protein of RBC)MyoglobinBence Jones ProteinsAlbumin and globulin are found in the urine in the condition called proteinuria, also in nephrotic syndrome and glomerular nephritis.hemoglobin is excreted in urinay tract infections.Myoglobin is excreted due to muscular atrophy etc.Bence Jones proteins are not usually synthesised in the body. They are produced in the condition called multiple myeloma, by plasma cells.KETONE BODIESthey are basically three chemicals viz.acetoneacetoacetic acidbeta hydroxy butaric acidAll the three are the intermediates in the fats metabolism.Increased catabolism of fats is due tostarvationincreased fat intake in dietdecreased carbohydrates in the dietdiabetes ketoacidosisRBC/ HEMOGLOBINiNhemolytic anemiaUrinary tract infectionsRenal traumaBILE SALTS:in JaundiceBILE PIGMENTS:in obstructive jaundice

Can asymptomatic bacteriuria in men cause bacterial vaginosis in women?

Asymptomatic bacteriuria in men will not cause bacterial vaginosis in women. Studies show that treating men has no effect on cure or recurrence rate of BV in their female partners.

Asymptomatic bacteriuria?

DefinitionAsymptomatic bacteriuria is a significant number of bacteria in the urine that occurs without usual symptoms such as burning during urination or frequent urination.Asymptomatic bacteriuria may not need treatment, which makes it different from a bacterial urinary tract infection.Causes, incidence, and risk factorsAsymptomatic bacteriuria occurs in a small number of healthy individuals. It more often affects women than men. The reasons for the lack of symptoms are not well understood. Most patients with asymptomatic bacteriuria do not need treatment because the bacteria isn't causing any harm. Persons who have urinary catheters often will have bacteriuria, but most will not have symptoms.Certain people are at a higher risk for kidney infections if they develop asymptomatic bacteriuria. The following increases your risk:DiabetesInfected kidney stonesKidney transplantOlder agePregnancy -- up to 40% of pregnant women with untreated asymptomatic bacteriuria will develop a kidney infectionVesicoureteral reflux in young childrenSymptomsBy definition, asymptomatic bacteriuria causes no symptoms. The symptoms of a urinary tract infection include burning during urination, an increased urgency to urinate, and increased frequency of urination.Signs and testsAsymptomatic bacteriuria is detected by the discovery of significant bacterial growth in a urine culture taken from a urine sample.TreatmentNot all patients with asymptomatic bacteriuria respond to treatment or even need treatment. Pregnant women, kidney transplant recipients, children with vesicoureteral reflux, and those with infected kidney stones appear to be more likely to benefit from treatment with antibiotics.Giving antibiotics to persons who have long-term urinary catheters in place may cause harm. The bacteria may be more difficult to treat and the patients may develop a yeast infection. If asymptomatic bacteriuria is found before a urological procedure, it should be treated to prevent complications. The course of treatment in these cases depends on the person's risk factors.Expectations (prognosis)Most individuals with asymptomatic bacteriuria who do not have risk factors for complications do extremely well. The outlook for those at high risk is good if the infection is detected early, but it depends on the person's overall health.ComplicationsUntreated, asymptomatic bacteriuria can lead to a kidney infection in high-risk individuals. In some cases, particularly in those who have had kidney transplants, such infections may lead to the loss of kidney function.Calling your health care providerCall your health care provider if the following symptoms occur:Difficulty emptying your bladderFeverFlank or back painPain with urinationYou will need to be evaluated for a bladder or kidney infection.ReferencesLin K, Fajardo K; U.S. Preventive Services Task Force. What are the symptoms of urianary tack infection?

In general, UTI signs and symptoms develop pretty fast and can include:• A strong, persistent urge to urinate• A burning sensation when urinating• Passing frequent, small amounts of urine• Blood in the urine (hematuria) or cloudy, strong-smelling urine• Bacteria in the urine (bacteriuria)Different parts of the urinary tract system can develop specific symptoms if infection appears:If infection attacks the urethra (urethritis), the most common symptom is burning duringurination. If infection enters to the bladder (cystitis), symptoms include pelvic pressure, lower abdomen discomfort and frequent, painful urination. If infection reach the kidneys (acute pyelonephritis), symptoms include upper back and side (flank) pain, high fever, shaking and chills and nausea. Discover more in recommended related link.

Signs and symptoms of hydrocele?

Symptoms of HydroceleMen who appear at a physician's office for the evaluation of a scrotal mass are frequently asymptomatic. It is not unusual to elicit an incidental history of scrotal or perineal trauma, which has prompted self-examination and discovery of the mass. The history of frequency, urgency, and dysuria associated with bacteriuria and of painful scrotal swelling suggests an inflammatory cause. There may also be pain in the groin or testicle. An accurate diagnosis can usually be made solely on the basis of physical findings. A complete examination of the scrotum - consisting of inspection, palpation, and transillumination - is made in every case. The presence of erythema and edema with loss of the normal scrotal rugae is suggestive of an inflammatory lesion (i.e., epididymitis or epididymo-orchitis.) In the presence of acute epididymitis, the epididymis is exquisitely tender to palpation and, in the absence of orchitis, easily separated from the normal testicle. Pain is aggravated by standing and should be relieved when the testicle is elevated (Prehn's sign).A smooth, cystic feeling mass completely surrounding the testicle and not involving the spermatic cord is characteristic of a hydrocele. A cystic, non-tender mass arising from the head of the epididymis and distinct from the testicle is characteristic of a spermatocele.
