This is a good question. As far as people know this ditty was made up way back when the Bubonic Plague was out in early America. It was believed that the posie plant could help cure this plague. The plague would kill so many people in an area that the only way to help stop it was to burn every thing the infected people touched or used. So... the ring signifies the amount of people it affected, the posie a hope of a cure, ashes for the way people sterilized the things the infected touched(including the dead infected bodies) and the grim reality of death from the plague in "we all fall down". Yeah... pretty grewsom nursery rhyme huh?
