When not in use, a dust cover goes over the ejection port. Additionally, the tolerances of the M16/M4s design assures a pretty tight fitting of the bolt carrier inside the receiver. There isn't space for dirt to get in through the ejection port while the bolt carrier is in the forward position. Additionally, service members lube their weapons lightly, as excessive lubricants will attract and hold dirt. Above all, routine maintenance is performed - weapons must be cleaned and maintained regularly.
