Only when things are intervening with the situation, i.e. his Hollow pops up, that Ichigo's bankai gets affected; this has been a thread-plotline for the duration of the bleach story, usually it's paired with some sort of power struggle on Ichigo's behalf as he develops and gets stronger... No, Ichigo's zanpakuto does not change in his hollow form. If you watch the "Zanpakuto Rebellion" arc, you can see this when Ichigo is fighting against the materialised form of Zangetsu, his zanpakuto. Ichigo cannot call on Zangetsu's power, and so he cannot use Bankai. Instead, he releases his hollow form, which is a raw power inside Ichigo. So Ichigo's zanpakuto remains completely unchanged while in hollow form.

After Mugetsu, Ichigo loses almost all of his Reiatsu right after the technique. He will then gradually and slowly lose the rest of his Shinigami Reiatsu in a month or so. It is possible to regain those powers afterwards, by developping your Fullbring (What Ichigo actually did :D )

Yes, as of 8/17/11, Ichigo officially gets his powers back. Since chapter 429 when he first meets the Fullbringers, he has perfected Fullbring which, at first, resembles his Shinigami powers. In chapter 459, Ginjo turns out to be evil and steals Ichigo's Fullbring abilities and whatever was left of his Shinigami powers (even though the Final Gutsuga Tenshou used all of it). Right as Ichigo gives into despair, Rukia shows up and stabs Ichigo with a Zanpakutou. (just like in the beginning of the manga). The sword was made by Urahara and had the Reiatsu of several captains and lieutenants including Toshiro, Renji, and Kenpachi. All of their combined Reiatsus give Ichigo the ability to re-create his own Reiatsu source. After a cloud of dust clears, Ichigo's newest form includes his old Shinigami uniform (with new elastic bands around his neck and wrists) and a ridiculously larger Zangetsu. In terms of power, Ichigo still retains his speed and strength. But in chapter 460 (when we first see him fight with his powers back) he seemingly uses Getsuga Tenshou at the same strength when he lost his powers, but it was actually him just releasing pressure from his sword. The chapter ends with Ichigo saying, "Getsuga...Tenshou." Thus inferring he is going to use a real Getsuga Tenshou. Hope this answers your question!
