Communication involves somebody speaking and somebody listening or somebody writing and somebody reading. So a breakdown of either part will make communication ineffective. For example if somebody writes you a note but their writing is so bad you cannot read the note then communication has broken down. Or if you are speaking to somebody but they are not listening then again communication has broken down or is ineffective.
The critical difference between effective and ineffective communication is the ability to connect with the audience. If the speaker fails to connect to their audience, having a good speech doesn't matter.

affective means that it has an effect on your body or it makes you cause reaction. ineffective means the opposite

When someone in the army made a plan and said its effective immediately they are saying they should do it right then and there. Then after someone gave that order if they decide not to do it it was ineffective.
