Updates are typically security fixes of some type or another. Some times they are "good" fixes and other times they are "bad" fixes. I prefer not to be bothered by the mess they can create and NEVER update my operating system. I use ZoneAlarm firewall and I pay attention to what it is telling me. (I don't let programs run that shouldn't be running, that I don't want to run, etc.) I have never gotten a virus or had any security issues. Some folks allow automatic updates and while some may never have a problem with it, most people I know end up with some sort of mess they can't fix and have to reload their operating system or take some other drastic measure to fix their computer.
(I'm assuming you mean antivirus software, not a virus.)
Your computer can get infected with new viruses that your antivirus software can't detect and stop because the database has not been updated.

A computer knows it needs a Microsoft update after it checks back with Microsoft during scheduled automatic updates or after a user manually enters Windows Update. If no updates are available, then nothing will happen, but if updates are available, users will often be given the choice of downloading and install a given update or ignoring it.
