The physical USB controller might be shot.
If the port is on the front of the base unit, check the able going from the port to the motherboad, reseat the cable at the mother board.
If the port is at the back of the base unit, built onto the motherboard, it may be a hardware fault, may need replacing.

If you have customized the USB into a mac it may show up in My Computer as a floppy disk or drive, I don't know how to change it back though, if this is the case, your USB should work fine on macs. Hope this helps :B

If 'Control Alt Delete' doesnt work and you are a client account on network you might not have it. That aside, goto C:/windows/system32 and look for taskmgr application. Once found you can re-establish the shortcut. If you don't find it you will have to try a reinstall through Control Panel 'Add/remove Programs' or attempt running an OS repair from the original Windows install disc.
