The difference between application-centric approach and document-centric approach is that with application-centric approach you open an application (such as notepad) and then create a document. With document-centric approach you create a document without opening an application first.
You can create a document from the desktop by right clicking on the desktop, then click on New. On the submenu click on text document (or whichever command you choose).
