First you must complete a certain quest- and also have completed the world before Marleybone. The character who gave you the quest was likely to be Headmaster Ambrose. Once you return to him, he will give you the spiral key for Marleybone.
Spiral fractures are described as crossing a bone at an oblique angle, creating a spiral pattern. This break usually occurs in the long bones of the body such as the upper arm bone (humerus) or the thigh bone
Spiral
Spiral bone fractures, or any fracures for that matter, could be caused by malnourishment of the bones. Weak bones=more breaks and fractures. Bone injury is almost always a result of trauma, however.

In Silent Hill 2, to unlock the door with the spiral key, you first need to find the spiral key itself, which is typically acquired by exploring specific areas and solving puzzles. Once you have the spiral key in your inventory, approach the locked door and select the key from your inventory to use it. This will unlock the door, allowing you to progress further in the game. Make sure to check your inventory and the environment for clues if you're having trouble finding the key.
I was level 30, but I don't know if it varies between quests or not.
A spiral fracture is bone breakage caused by a twisting force. They are also known as torsion fractures. Treatment can be months in a cast and possible surgery. The classic example is when skiers lock their boots into the ski and then lose control, the leg may be violently twisted in one direction.
In a femur it would be something like a hard landing on your foot causing pressure and thereby creating a spiral as it cracked in an upward motion. In an arm is similar. It takes a lot of force to cause a spiral fracture.
