It may be due to bad sectors that cannot be repaired, or depending on the format partition type FAT16, FAT32, or NTFS. It may be you have the physical HD partitioned into logical drives.
Single Sided, Double Density This is a floppy disk format.
either your hard drive is going bad, or your doing a full format witch writes all 0s to the hard disk if you do a quick format it just deletes the registry and the information is over written. It is always better to do a quick long format
an easy way is to go to my computer and right click on the usb drive. then click format... after that you can set it either for quick format or full format. make your choice and click OK/continue or format. then after it finishes your usb will be completely empty
A quick format simply erases the file control structures on the partition such as the Master File Table (MFT) on NTFS formatted partitions or the File Allocation Table (FAT) on FAT(32) formatted partitions. A full format also checks the drive for bad sectors and, upon encountering any, adds them to the drives internal list of bad sectors so they will be completely ignored and unused by the file system. When formatting a new disk for the first time or a disk with unknown history it is better to perform a full format. If you are just reinstalling your operating system on a known good drive then a quick format will suffice.
You need to format it, and check for errors (full scan including the disk surface). After you can just use it.
You go to My Computer and right click Floppy (A)and click Format. A dialog will appear and after selecting the right options for you (quick/full format, label etc.) you click Start.Note: A floppy disk must be in the drive A: for this to work.First you are going to need a command prompt. From the start button win7 in the search field type (cmd). Then from the command prompt type format A:

You have to find out where the BOOT.INI is. You might have two of them. If you have two of them you can format the d drive. If you have one on the disk d you have to use restoring CD to put BOOT.INI file on disk C. Again if you have BOOT.INI on c you can format d.
