If you are trying to create an anonymous survey I would suggest posting it on a blog and then use an anonymous messaging service to acquire the data. You basically send the message to a remailer, who hides your identity, by erasing all traces of your passage. It is a safe way to stay anonymous. Great idea.
The quick answer is NO. But yes, you can send a survey and technically collect the data anonymously. But- if you mean truly anonymous- it is not, because you have to send the survey out to personal email addresses or if they come to survey monkey they technically leave behind a digital trail.
