To smooth out skin, use the smudge tool. It's the hand icon near the bottom of the icons in the tool box. Select a circle brush, preferably one of the blurry ones, and change the rate to 20 or less. Then go to your picture, click and run the brush over the skin to smooth it out.
To change the lighting, go to colors >brightness/contrast and adjust the levels to your liking. The image should change as you change the settings. When you're happy with the results, click 'ok'.
