I have to say, the post painful trap is the rack from saw III. it twists your limbs,one by one, beyond it's possible motion. the trap then twists your head 180 degrees.
the most disgusting trap is the pig vat. i just dont want to drown in pig remains.
Honorable mention: needle pit. people with trypanophobia (fear of needles) will definitely avoid this one. especially because it's filled with used needles!
